RE: Sexual Harassment Prevention Training
REACH Community and Youth Agency is committed to maintaining a workplace free from sexual harassment. REACH CYA has a sexual harassment prevention policy in place that applies to all employees, paid or unpaid interns and non-employees in our workplace, regardless of immigration status. New York State Law requires that employers of one or more employees must conduct anti-sexual harassment training for all employees yearly.
The Training will consists of 3 parts:
- Read REACH CYA’s Sexual Harassment Policy
- View Training Video
- Complete, sign and return the New York State Sexual Harassment Prevention Training Assessment Form. (This form is used to accompany New York State’s Sexual Harassment Prevention Training video. While taking the training, please note your answers to the case studies and exercises).
- This form is available in paper form (available at each site)

3. Complete, sign and return the New York State Sexual Harassment Prevention Training Assessment Form. (This form is used to accompany New York State’s Sexual Harassment Prevention Training video. While taking the training, please note your answers to the case studies and exercises).

Steps to complete, sign and email form with iphone: (for android users you will need to rely on google, youtube, or any 5 year old you may know)
- Click onto link
- Tap Upload Arrow on top right
- Tap View More on Bottom right
- Scroll down page to “Save to Files”
- Tap Save on top right
- Close out of screen
- Tap on Files App (Blue)
- If you don’t see Sexual Harassment Sheet, tap onto “Recents” on bottom of screen
- Tap onto File
- Complete all questions
- Click into grey boxes and fill in Name and Date in boxes at bottom of page (typing your full name in the box serves as your electronic signature for this document and also acknowledges that you have; viewed the video in it’s entirety and read REACH CYA’s Sexual Harassment Policy).
- Tap Done at bottom right
- Tap on 3 dots … on top right
- Scroll down page to “send a copy”
- Tap onto your email app
- Send to Rmiceli@reachcya.org
- Hit arrow on top right to send.
If you completed the online form on a desktop or laptop, simply save it and attach the document and email it to Rmiceli@reachcya.org
If you completed the paper form, you can either:
- Scan and email both sides of the form to Rmiceli@reachcya.org
- Take a clear photo of both sides of form and email it to Rmiceli@reachcya.org.
- Mail form to Att: Roseann Miceli, Executive Director, REACH CYA, 525 Half Hollow Road, Dix Hills, NY 11746.
- Inter-office the form to R. Miceli @ REACH CYA
