UPK Program Class Assignment & Student Intake Form

We have sent your UPK Program Class Assignment to the email you had supplied the school district at registration and have not yet received your completed Student Intake Form that was due earlier this week. If you do not see the email, please check your spam/junk folder.

REACH CYA’s Student Intake Form is separate/different than the information you supplied the school district at registration.  

Please complete the Student Intake Form as soon as possible so that we can complete the necessary paperwork for our Teachers.

If you have decided not to attend REACH CYA’s UPK program with Half Hollow Hills School District, please email us at info@reachcya.org so that a family can be contacted off the waitlist.
